- Opłata za wydanie karty pobytu (card issuance fee) — PLN 100. Paid to the account of the Urząd do Spraw Cudzoziemców (UDSC). Separate account, separate transfer.

The Photo, Passport Scan, and Health Insurance Details Everyone Gets Wrong
Three smaller items — but they each generate real errors inside MOS.
Photos: MOS requires 4 biometric photos, 35×45 mm, white background, face uncovered, taken within the last 6 months. The photos must be scanned and uploaded as a JPG (max 10 MB per file). A selfie or a photo taken by a phone directly doesn't count — take them at a photo shop and ask for 'zdjęcia biometryczne'. The photo also needs to be physically submitted at your biometrics appointment.
